An Approach to Personalized CareI’ve previously talked about the importance of proactive care to prevent hospital visits for people whose health requires in-home healthcare. So, what’s the best proactive approach?

That would be a whole-team ecosystem coordinated by one person—a person who is entrusted to provide the best care for that loved one. “Entrusted” is an essential component. I understand that many offspring think they know their parents the best and, therefore, know what’s best for them. Or there are physicians who see the patient periodically and think they know the best course of action for treating them.

However, your home-bound loved one needs a combination of the above: Someone with healthcare knowledge who is dedicated to them and with them consistently enough to see little signs of distress. That’s why in-home healthcare in combination with a concierge physician is such a powerful approach. A caregiver will have the personal connection to notice little things that foretell trouble, and the concierge physician will know your loved one well enough to know if those signs are new and concerning. Concierge physicians can direct the care, including bringing in specialists, and work on your loved one’s schedule. By bringing the caregiver together with a concierge physician, you create personalized service for the best care.
